Advance Health Care Directive

An Advance Health Care Directive is also called a Living Will and nominates an individual or individuals to make health care decisions for you. Your nominated health care agent makes sure that health care providers follow your wishes. You can restrict the power of your health care agent or you can allow your agent to decide all medical treatments and procedures for you. This document also outlines your decisions regarding end-of-life care such as electing palliative care.
